They get her and pull her into an unseen dimension.

The alternate ending better explains what is actually occurring within Julia's mind. She has paranoid schizophrenia. However, it is never explained what 'They' are, but more than likely the creatures which dwell in the dark are metaphorical and the bleak black hole often represented by schizophrenics is reaching out to take hold of her. The audience only realises this when Julia awakens in a mental hospital, but it is unknown whether she is herself aware that the events of the movie played out within a prolonged hallucination.
