Trivia:

This was Ultimate Warrior's final appearance at Survivor Series. He disappeared from the WWF after Summerslam 1991 less than a year later, and his two ensuing returns to the WWF did not include appearances at the Survivor Series.

Goofs:

Continuity: The Undertaker was counted out in the Dream Team vs Million Dollar Team match even though he wasn't the legal man, but Rick Martel was not counted out with Jake The Snake Roberts in the Vipers vs Visionaries match when Undertaker was counted out under similar circumstances during the previous match.
