Following his title win, Robert Remus had to remain at the arena until around 3am amidst fears for his safety. At the time, Slaughter's in-ring persona was that of an Iraqi/Saddam Hussein sympathizer and as this event took place during the first Gulf War, fans were incensed when he captured the title.

The promotional poster of the 1991 Royal Rumble served as the inspiration for the promotional poster of the 2001 Royal Rumble which featured the top WWE Superstars in a similar format. Ironically, this poster would be released 10 years after the 1991 Royal Rumble.

This marks the only time in Royal Rumble history that a referee attempted to count out a wrestler outside the ring in the Rumble match even though count outs are not part of this match. Ironically, it was Shane McMahon (as a young ref) who made the mistake, attempting to count out Rick Martel, who refused to get back in the ring.
