Many of the male characters smoke tobacco. The character of George Grossmith, played by Martin Savage, is seen injecting himself with opium in his dressing room. The character of Leonora Braham (Shirley Henderson) is shown being warned against drunkenness, and is visibly drunk in the last scene.

As the Mikado begins, Sullivan (Jim Broadbent) walks through an unsavory side of London, and a drunk or confused woman accosts him: menacing music is played.
