Revealing mistakes
The hotel computer displays the message "ESTE HUESPED HA SALIDA" under "THIS GUEST HAS CHECKED OUT". There is both a spelling mistake, the correct Spanish being "ESTE HUESPED HA SALIDO", and a grammatical error: this would translate to "this guest has gone out". The correct translation should be "este huesped se ha ido".
When Angela watches the news about Jeff Gregg in her motel room, she is watching a Sharp brand television. When there is a close up of the TV screen it is a SONY, in the next wide shot the TV is a Sharp again.
When the paperwork for the temporary visa is initially revealed, it enumerates Ruth Marx's birthday as March 5, 1968. But when the police consult their database for Wants/Warrants on Ruth Marx, their system reports Ruth Marx's birthday as June 11, 1968.
When Angela goes to the Cathedral office building, you can see floor 20 on the elevator doors as she gets off yet later when Marx is calling security she says '5th floor Cathedral'.
There is no scar on Jack's face from the cigarette on the boat.
When Angela knocks Devlin over the railing with the fire extinguisher, she lets go of the extinguisher as well. Yet the extinguisher never falls with Devlin, it is just shown in the next clip already on the floor.
When Angela visits Alan Champion in hospital, she is told that he is intubated. When she goes to see him, he is sitting up in bed - no tube down his throat.
Right before discovering Bergstrom's mis-diagnosis of Aids, she connects to a system using the telnet command with an email address that was written on a card she had taken from Devlin's wallet. It is impossible to establish a connection using telnet with an email address. The only way to connect using telnet is with a FQDN (Fully Qualified Domain Name) or IP address (and/or a TCP port number). Once the telnet connection is established, you can however login to a system using an email address.
When Angela is presented with the application for a temporary visa under the name "Ruth Marx," there should be a replica signature on it for purposes of identification; even in 1995 California would have had signature facsimiles on file.
The Macintosh virus Angela found would not be able to infect a mainframe computer.
When Angela is driving in the BMW on the Highway with the police car behind her, the raindrops on the side window are going down on it vertically, but they should be blown a little backwards by the airstream. Also the raindrops on the windscreen of the police car should go upwards to the roof because of the airstream, but they just run down on it.
When searching for Praetorian, Angela searches for the owner of IP address: 24.75.345.200 This address would be impossible on the Internet because no subnet address can be greater than 255. This has been reported as a goof, but it was surely deliberate by the filmmakers. It would be very poor practice to show a genuine, usable IP address, because the present or future owner of that address would undoubtedly be subject to massive flooding, ranging from spam to actual malicious hacking attempts. This writer noted the same fact at "Swordfish" in the Trivia section, http://www.imdb.com/title/tt0244244/trivia.
The visa paperwork reveals Ruth Marx's birthplace as California. As she was born in California, she would be a US citizen and would not need a visa to re-enter the US. But she would need valid ID showing her citizenship, and she has already claimed the driver's license she's carrying is a fake.
Angela claims to be in Cozumel but when she returns to the hotel from the hospital and insists that she hasn't checked out the computer clearly states that the hotel is in Cancun. Although an island, Cozumel is in the Cancun area, and resorts there commonly show a Cancun address.
When Angela takes off the stolen firefighter uniform, she has on a jacket that she wasn't previously wearing. But it can be seen tied around her waist when she enters the cathedral office.
A gun goes off with such a loud report it frightens a flock of birds into flight, but a nearby jogger continues running on as if he hadn't heard anything untoward.
It is possible the jogger is deaf or hard of hearing, or is simply in the zone.

The drive on Dale's desk while he is talking to Angela is not hooked up. It has a power cord, but the SCSI data ports do not have cables hooked to them.
In the opening scene, Secretary Michael Bergstrom asks his driver, Eddie, to take the parkway. The Lincoln Towncar Eddie is driving is in park, and the ignition is in the off position.
When Dale is on approach to Burbank he says he is intercepting the approach course ILS runway 8. He then reports just passing the middle marker which is only 0.4 miles from the airport and way too late for him to be intercepting the final approach course. He reports at 1300 feet (he should be at 1000), but the altimeter shows 620 feet which is below the airport altitude of 775. Also, the GPS shows him still 35.4 miles away from the airport. (Additionally, the entire approach is conducted in visual conditions where he'd be able to see the approaching towers from a long distance away.)
When Jack cuts his hand with his pocket knife to pretend he's been attacked, the blade of the knife is facing upwards, and Jack is cutting himself with the blunt part of the knife.
When Angela calls Russ, she presses the "Dial" button and then the "RLS" (Release) button, and the phone starts speed dialing. On this brand of phone (Nortel), pressing "RLS" would hang up a call (she does properly press "RLS" again at the end of the call). In order to initiate a speakerphone speed dial on a Nortel, you must press a memory number then press "Dial".
After Devlin unwittingly unleashes Angela's virus into Gregg's mainframe, one of the pages seen shows the hacked personal details for Dr. Alan Champion. Under his personal data it shows that his car is a blue 4-door 1994 Mercedes 300S. However, throughout the movie his car is a black 4-door 1995 BMW 525i (E34 model) and not a Mercedes-Benz.
After Angela escapes over the rising bridge, it stops rising. There's also no boat approaching.
Angela closely embraces Devlin on the beach and again on the boat, after which he is soon shown disrobed. Angela has on each occasion failed to notice the pistol inside his jacket. And no, he wasn't just happy to see her.
When Angela is supposed to meet Dan at the Ferris Wheel, it is set for 7:00 pm. It should not be that dark at that time of night.
The gun used by the bad guy on the boat clicks several times as he repeatedly pulls the trigger. This gun is a Glock, which requires movement of the slide for recocking. Hence, only one click would be possible.
After Angela has set off the fire alarm at Cathedral and made the others leave, she checks the computer. She hears the fire engines outside, and walks over to the window. As she walks, we can see the camera rig reflected in the cubicle wall glass.
There are no mountains in Cozumel, as shown.
During the opening sequence, when the Secretary is being driven in his car they are proceeding down Pennsylvania Avenue, but when the camera cuts to the view from the right side of the car there is a large expanse of water to the left. This would be impossible, as Pennsylvania Avenue is in the middle of downtown Washington DC.
Angela sleeps overnight in Alan's car, is woken in the morning by the security guard and she drives away with the Pacific ocean on her left. But as she drives northbound, the shadows indicate the sun, also on her left, is setting, so it must be late afternoon, not morning.
The scene with FBI Agent, Ben Phillips, was clearly filmed in/near Alameda, CA. However, the caption on the KTVU newscast lists "Live in Watsonvile" (almost 100 miles away from Alameda).
Even in the 21st century, computer records also have paper "backups". While computer records could easily be changed, the paper records could weeks or eve months to change, if they could even be accessed.
Simply placing inaccurate information in databases would cause minor headaches for most people. The harm shown in the film could be corrected within a few hours or a day at most.

Unclear why the ruthless, murderous, and presumably strategically intelligent Praetorians chose to play elaborate, time-and-labor intensive psychological mind games with Angela, rather than saving themselves a lot of trouble by killing her during her relatively unguarded stay in hospital. If they had, they would also have seen the destroyed disk left out on her nightstand. This even though they immediately murdered every other person who came close to being aware of their existence. Including other hackers who could themselves have been sent and made copies (and been better at keeping important activity offline). So the explanation that Devlin and co allowed her to return to California, and kept her alive, because they want the disk and need to know if she made any copies is extremely thin.
To be fair, they do make one murder attempt in the beginning, and several more after, but it's odd that they would not think to ask about other evidence potentially being out *before* that first attempt.
Unless they have some sort of fatal superstition about doing their business in a religious hospital. Which still wouldn't explain why they let her leave Cozumel at all.

As a practicing doctor, Dr. Champion would have known that Seldane was the first non-drowsy antihistamine (so, not much fun to add to a cocktail), and that it was known to trigger heart problems from 1990 when the FDA required the manufacturer to mail a letter announcing that fact to all US doctors. Within 2 years, the warning was printed in the drug's packaging, so by the time this film is set, he'd be aware of the danger. By 1998, it had been removed from the US market.
The hotel computer displays the message "ESTE HUESPED HA SALIDA" instead of the correct one "ESTE HUESPED HA SALIDO"
Angela does not leave the building when the fire alarm goes off.
