When the characters travel to Puerto Rico, during the night before the final events, Miguel asks two women vendors at the "El Dia de los Muertos" (Day of the Dead.) festival if they spoke English. He speaks Spanish earlier in the plot though he seems to forget how with Spanish speakers when in Puerto Rico.
In the scene where Miguel is in the tower and setting up to shoot Rath coming out of the bank, with his left hand he pulls a bullet out of a holder that is strapped to his right wrist, yet when he loads the round in the weapon, he loads it into the chamber with his right hand. Him having the bullet holder strapped to his right wrist would be also be an incorrect procedure, as he would have to pull the bullet out with his left hand, then switch the bullet to the right hand to load and close the bolt. This would take up too much time.
When Rath and Elektra first drive up to the old hotel in the station wagon taxi, after they get out Elektra is heard saying "keep the air on". When they drove up, the windows in the car were open, and both were sweating. They wouldn't have opened the windows if the a/c was on.
The table that Bain picks up and shields himself with when the apartment explodes disappears after the first shot. Also, in later scenes his hands (especially his fingertips) appear uninjured even though they were exposed to the damaging effects of the explosion.
Electra parks the Mustang up close to a chain link fence and jumps out, slowing Rath's pursuit. This occurs in bright sunlight. Barely a minute later, Rath returns to the car with Electra's cat, the car and fence are in the shade of a building.
(At 1:13:05-1:16:00) Rath originally gets on the Seattle monorail at Westlake Center terminus, makes the exchange at the Seattle Center terminus and gets back on the monorail. He jumps off on to the track buttress by exiting the right side of the train (in correlation with the train's motion). This means that the train is going in the wrong direction as the buttress is on the left side when traveling from Seattle Center to Westlake Center. Further proof is the exterior shot of the monorail heading toward the Space Needle after it resumes operation.
Nicolai Tashlinkov tells Robert Rath that he faked his death back in 1980 because "the Cold War was ending". In reality, the Cold War was not just NOT looking it would be over at that point, but one of its most contentious and dangerous periods of dispute between the U.S. and the Soviet Union began around at that time. Any discussion of when it seemed like the Cold War could even be settling down (let alone completely and permanently ceasing) would be ridiculous before Mikhail Gorbachev became the USSR's top leader, and he didn't do so until March 1985.
In Miguel's escape scene in the back of the police cruiser, after he breaks the drivers neck, in the next scene you can see the driver driving the car, whereas in the next shot it's the second cop in the passenger seat who has control of the wheel.
Incorrectly regarded as goofs: When Bain has killed one cop and crashed the police car, he uses one of the police sniper rifles to shoot the cops in the car that's driving towards the crash scene, which causes a car crash that kills the cop who was injured and trapped but alive. It appears at first that Bain shot both of the cops who were driving towards him to death, which would make it impossible for the car to have driven that fast over the distance where it would have hit the first car. But while it's clear that Bain's shot at the female cop in the front seat who isn't driving hits her in the head and immediately kills her, the scene doesn't make clear where the cop who was driving got shot (forehead, chin, upper body are possibilities), so it's also unclear if that would have been a fatal wound. Also, Bain wants the trapped cop dead so he can't be a witness, so he could have used his skill to seriously injure the driving cop and then cause the crash which killed all of the cops before he left the area.
After the funeral shooting scene, when the police arrests Miguel, they just let everybody go, including Robert. This would be highly unlikely since it is a crime scene and they are all witnesses to a murder. They would at least be contained for questioning.
During the fight scene at Electra's apartment, Electra is escaping and drives past her neighbor a couple of blocks away, who waves at her. As soon as the neighbor gets to her apartment and discovers a hidden camera, she rushes to confront Electra at her home, but she had just seen her driving away and would have known she would not find her there.
