Michael J. Fox was diagnosed with Parkinson's disease in 1990 not long before the filming of this movie began, when he noticed a small but persistent tremor in his left pinky finger.
This movie was filmed in Micanopy, Florida. A small town of 650 people just south of Gainesville.
While in the restaurant in LA, Nancy Lee asks Hank (Woody Harrelson), "Isn't that a star?" He replies, "No, that's Ted Danson." Danson was Harrelson's Cheers (1982) co-star.
The writer, Neil B. Shulman, worked at Grady Memorial Hospital in Atlanta, Georgia, hence the name of the town, Grady.
The car driven cross-country is a 1956 Porsche 356 Speedster.
At one point Hank Gordon says, "I don't trust a man who doesn't eat meat." In real life, Woody Harrelson is a vegan.
The city Michael J. Fox is driving through in the opening credits is Richmond, VA.
During the Squash Festival, the large audience is watching The General (1926).
When Ben ( Michael J. Fox ) meets Hank ( Woody Harrelson ) in the café, Hank asks Ben about getting life insurance " Or are you too chicken?" A reference to Michael J. Fox's character in Back to the Future II(1989) not being able to back down when asked if he was " chicken? "
You never hear Melvin's assistant, Lane, speak in his normal voice except while looking at the car after running over the fence.

First musical collaboration between Michael Caton Jones and Carter Burwell. Last movie they did together was The Jackal made 6 years after this film.

When this film was first released on VHS in late 1991, two previews that preceded the start of the movie were for Danny De Vito's film "Other People's Money (1991) and for Christina Applegate's film "Don't Tell Mom the Babysitter's Dead" (1991), both of which were released the same year as Doc Hollywood. Michael J. Fox the main star of this movie would go on to co-star with both De Vito and Applegate in Tim Buron's sci-fi comedy "Mars Attacks!"(1996) five years later.
