When Larry is talking about drugs, his lip movements don't match his voice.

When Larry is commenting on Bernie's drug habit, with his back to the camera, he says, "I watch 20/20, I know the statistics." When he turns his face to the side, he is still speaking, but his lips aren't moving.

As Larry and Richard are walking past one of the guards in the beginning of the movie, the guard asks if its hot enough and Larry replies. You don't see either one's lips move.

Continuity 

The recording of Bernie's and Paulie's phone conversation is different from the original. Paulie's "Uh huh!" is missing.
When Bernie falls off the boat, Larry turns around just in time to see him go. When he looks back a few seconds later, he's surprised that Bernie isn't there.
When Gwen and Richard are lying on the beach kissing, a wave washes up and wets Gwen's hair. When she sits up, her hair and clothes are dry.
When Larry and Richard first arrive at Bernie's beach house and talk to the lifeguard, there is no wooden walkway visible beside the house. At the end, a wooden walkway has appeared.
On the rooftop at Larry and Richard's workplace, Larry sits in an inflatable pool, takes his socks off, and throws them on the ground. The right sock moves, and disappears in one shot.
When Richard and Larry are on the roof Larry picks up the radio. He gets tar on it. A couple of minutes later when Richard comes over to explain the error, there is no tar on the radio.
When dealing with the corpse inside the house, Richard alternates between shod and barefoot.

When Paulie is in the house and he's chasing after Richard, Larry, and Gwen, Gwen's hat disappears from her hand and is later seen lying next to Bernie's body.

When Larry goes to make coffee in the kitchen, Bernie's head falls on Richard's shoulder. In the next shot, Bernie's head is upright, and falls on Richard's shoulder again.

At the restaurant, Tina eats the olive from her martini. In the next shot, and for the rest of the meal, the olive is back in the glass.

When Larry and Richard arrive at the ferry to Hampton island, the ferry moves quickly when they jump on. In the next shot, the ferry is at a dead stop.

When Gwen is in the building's lobby for the last time, Larry is holding a cigarette. When Larry and Richard follow Gwen onto the elevator, Larry's cigarette is gone.

Bernie is dropped over the side of the deck, next to the house and is later swept out by the tide. Later shots show a dune between the ocean and the house, revealing that the tide does not come up that high.

When the ambulance bumps the boardwalk causing the stretcher with Bernie on it to roll down to the beach, the stretcher is empty when it is bumped by the ambulance.

When Richard confronts Larry about throwing Bernie's body over the deck he is barefoot. When he subsequently goes to call the cops, he is wearing shoes. But then when he runs outside onto the deck and then the beach with Larry to retrieve Bernie, he is barefoot again. Once he and Larry bring him back inside of the house to vacuum sand off of him, his shoes are on once again.

When Larry, Richard, and Bernie are in Bernie's office discussing the missing money, Bernie walks over to the bar, drops the paperwork on the counter, and opens the left door completely. In the next shot, the left door is closed.

While sitting on the sofa with Richard - around the time Larry and Richard discover he is dead, Bernie's legs are splayed. Several seconds later his legs are crossed.

Factual errors 

When Larry and Richard are getting lunch at a food stand, they see Bernie entering a building with the entrance awning reading East 70th Street. When Larry and Richard begin walking down the street, a sign behind them reads 52nd Street. Larry and Richard must have had extremely powerful vision if they were able to see Bernie from 18 blocks away.

Incorrectly regarded as goofs 

Richard is shown wearing a wedding ring during the movie. He isn't married as he is trying to date Gwen. However, he may have once been married but then his wife died or he got divorced, and he kept the ring.

Revealing mistakes 

When the 'boys' are transferring Bernie from the golf cart to the boat, you can see Bernie's hands clenching and opening.
When Larry and Richard are in the empty office on Saturday, you can see that the computers on the desks are missing their power cords.
Bernie occasionally blinks his eyes, and otherwise acts alive.
