Yes, it was based on Ron Kovic's autobiographical book, Born on the Fourth of July.

Probably because he believed that he'd one day walk again, which never happens. Also, he probably felt it was more important to live as a whole person rather than lose a limb that could be repaired. The largest aspect of Ron's injury is that the doctors could fix it surgically so Ron could keep it, however, as we saw from the deplorable conditions in the VA hospital, the money to do so was scarce.

They were that bad and worse. Because the war effort was costing the United States government so much money in equipment, supplies, missions, etc, there was little money that was budgeted to take care of the soldiers who returned home. Kovic talks about the hospital he lived in extensively in his book.

The officer didn't want the incident to be associated with his unit or his own reputation. An incident like this one could hurt the morale of the unit and Ron could easily be ostracized. Additionally, the officer would have had to answer to an investigation of the shooting which could damage his reputation with his superiors.
