Revealing mistakes 

Shortly before Matrix kills Arius, he fires his assault weapon through multiple glass window panes, shattering most. However, no parts of the window frames are damaged, although when Matrix next jumps through the shattered windows, it is clearly seen that the frames are made of wood or an easily breakable material. It would be impossible for Matrix's bullets to have hit the glass windows only with no damage to the delicate framing around them.

Character error 

Matrix, a former Army Special Forces commando, secures his grenades to his tactical vest by tying the pins to a metal loop on the vest. No soldier with even a minimum of combat training with ever do this, due the hazard of the pin being accidentally pulled out, and killing yourself.
When Matrix is surprised by the barking dog in the cage on the airliner, the dog keeps barking at something else and pays no attention to Matrix as he moves away from the dog's cage.

In shots of the seaplane near the island you can see the struts of the landing gear indicating that the gear is extended to the down position. However, in a water landing the wheels would remain retracted. The wheels would only be extended if they planned on taxing the plane up a ramp onto dry land.

At 52:05 John Matrix looks at the coordinates which are N33°13' W119°18' and says "these coordinates are somewhere near Santa Barbara. Those coordinates are nowhere near Santa Barbara, they're at San Nicholas Island, 87 miles (140Km) off its coast. However, when he finds them on the map, he points right to San Nicholas island.

The airstairs at Val Verde Airport say "Aire Servicio", which is an incoherent, word-for-word translation of "Air Service" into Spanish. (Translated back into English, it would mean something like "Service. Skies.") The correct translation would have been "Servicio aéreo".

Continuity 

When Matrix enters the shopping mall, it's daylight. When he leaves only a few minutes later, it's dark outside.
The yellow Porsche is damaged in the side collision with the red convertible, but when Matrix flips it upright and drives off in it, the side is undamaged.
When Matrix is raiding the surplus store, and enters the hidden room full of guns, he grabs the rocket launcher off the wall and hands it to Cindy. After he says "Go!" and she leaves the room with the rocket launcher, the next shot of Matrix shows it hanging on the wall again.

During the big shoot-out near the end of the film, Matrix is using an M-60 machine gun which repeatedly acquires more ammo (the ammo belt is replenished) even though Matrix has no spare ammo with him, nor any time to reload.

At the end, when the helicopters and the sea plane land at the beach, you can see a cruise ship in the background in a wide shot. A later wide shot shows the pier and open sea, but no ship. Then in a final shot, you see the pier and the cruise ship.

When Matrix bulldozes the entrance to the surplus store, there is debris blocking the entrance. Cindy needs to step over this debris on her way, yet is able to roll a shopping cart full of weapons and ammo out the same path on her way out.

When Matrix jumps from the landing gear of the plane, he lands in water and gets his trousers and some of his jacket wet. Although he takes his jacket off, in the next scenes, his trousers are completely dry again, only a minute or so after getting out of the swamp.

When Matrix is "shopping" he puts many guns in the cart but he grabs the pistol and the shotgun shells after he tells Cindy to leave with the cart. Then he is arrested and the weapons that he grabbed after Cindy left are obviously confiscated, yet when he suits up on the beach for battle the pistol is one of the weapons he has and during the shootout the shotgun is fully loaded.

At the beginning of the movie when Arius' henchmen shoot at Matrix's home, Matrix has a cut on the side of his forehead which keeps appearing and disappearing in between shots while he's talking to Kirby's soldier, Jackson.

Matrix is holding a G3 assault rifle, but when he shoots the thug it is clearly a pump action shotgun in his hand. In the next shot Matrix is holding the G3 again.

A second Blazer (a base 1973 or 1974 model) is used during the shoot but has a basic gray grill (1973/74 Chevrolet C/K series), white front hubs and much smaller front tires.

Factual errors 

When Matrix escapes from the aircraft during take-off, he leaves doors open and breaks down sealed curtains. This would set off alarms in the cockpit and leave the aircraft unpressurized in flight; and therefore unable to complete an 11hr flight.
When Matrix crashes his car while chasing Sully, neither Matrix nor Cindy are affected by the crash even though neither are wearing seat belts (Cindy isn't even in a seat!), and the crash was powerful enough to severely damage the front end.

When Matrix is following the kidnappers with his sabotaged truck he goes down the forest in a straight line. The guys he's chasing use the road, which has several turns. He would actually overtake them after the second turn. Yet every time we seem him he is about the same distance away from the cars he's chasing.

When Matrix jumps from the plane is about 500 feet in the air. He lands in water about 3 feet deep and stands up unhurt, and is only wet from the waste down.
How is it that the two cops in the paddy wagon are incapacitated by the rocket, but Matrix is able to walk away unscathed? I realize it's Arnold, but...

The scene where Arnold is nearly killed by a hand grenade. The grenade is an M-67 frag grenade. It has a kill radius of five meters. That's roughly 15 feet. It explodes just as he is diving over a bush. The blast appears to be much closer than fifteen feet from him. In reality, he would not have survived.

When Matrix gets his weapons from the shed during the attack that ends with his daughter being kidnapped, we see him pull back the loading handle on his rifle - however, the weapon is a Heckler & Koch HK91 which fires from a closed bolt, meaning that to make it ready to fire you actually have to slap the cocking handle into the forward position (after inserting a loaded magazine). Note: It's the same action you see Keanu Reeves perform on a H&K MP5K submachine gun (basically the compact submachine gun version of the H&K rifle) during the "lots of guns" scene in... The Matrix.

Towards the end of the movie when General Kirbys female enlisted clerk comes in and says "General Kirby, you'd better get out here, we have your message on the teletype", this would never happen, as nobody would ever tell a general "you'd better get out here "unless it was another General speaking to them.

A flight attendant tells Matrix that the plane will land in Val Verde in "exactly 11 hours" and makes no mention of a refueling stop somewhere en route, either during that conversation or in the public address announcement made moments before. However, the aircraft in question is a McDonnell-Douglas DC-10-10, which has a maximum range of 3,800 miles or about 7 hours.

In the opening scene, Matrix is in the woods in a wife beater, but as he approaches his house, all the chimneys are pouring smoke as if there's a fire in each fireplace (or the furnace is going), which would indicate that the weather is cold. Jenny is also dressed in short pants.

Plot holes 

During the fight with Cooke in the motel, Matrix mocks the fighting abilities of the Green Berets, which is the nickname of the U.S. Army's Special Forces. Everything in the film indicates Matrix himself is a former Green Beret, including wearing an Army Special Forces belt buckle.

In the fight scene in the motel room, Matrix grabs Cooke's arm and forces it through the glass tiling in the bathroom. The tiles wobble and fall away without resistance and in a closer shot you can see that there is no cement or other bonding material holding the individual tiles together.

In the fight between Matrix and Arius, Matrix rolls out and fires his shotgun repeatedly at Arius leaving blood stained bullet-holes scattered randomly all over Arius' chest. As Arius falls towards the window, the bullet holes can be seen all lined up in little sets of six.
