James Cagney was originally supposed to play Nick, but when The Public Enemy (1931) became a big hit, it was decided that he should no longer be relegated to supporting roles, allowing the relatively unknown Clark Gable to step in instead.
In an earlier draft of the script, intern Eagan planted the skeleton in a baby carriage, not Lora's bed. The startled Laura dropped the baby she was holding, with tragic results. Eagan admitted to his guilt and was fired from the hospital. However, the sequence was changed to the one which is now appears in the film.
Ben Lyon and wife Bebe Daniels leased William A. Wellman and his wife a house that they occupied during the course of shooting.

The traffic accident gag at the end is actually the same stunt shown twice, with only the soundtrack altered.

When Nurse Maloney is talking with Mrs. Maxwell and waiting to be relieved, she spends no less than one minute and 55 seconds putting on a single glove and adjusting its fit. Obsessive and nervous behavior is inconsistent with the confident nature of the character. Perhaps the actress or the director felt she needed to be doing something with her hands.
