Disturbing/Horrific but Intelligent Movies

Film can be a powerful weapon in the right hands or it can become a horrific mess in the wrong ones. That being said, I have seen THOUSANDS of movies in my 40+ years. There are far too many that were immediately forgettable. BUT... there are a small handful of films I will never forget, or cannot forget because the images or content made me change my mind about what should or should not be put on film.
This is my (Incomplete) list of movies that are definitely NOT for the average film viewer! these movies had one or more elements that really made me uncomfortable, but were NOT merely gore-filled or gross-outs just for the sake of being disgusting.
There are several movies that I deliberately left off the list such as the "Guinea Pig" series for various reasons. Usually because they simply didn't fit the criteria I used to make this list. More often because they were made to simply gross out the viewer.
These films are (for the most part) intelligent, well directed and well written with above average to extraordinary acting that just happen to include extreme violence, depravity or other disturbing themes that will probably stick with the viewer long after the end credits.

The violence is almost too realistic and the motivations of the "bad guys" almost seem legit if you think about it. Some people hated the ending, but it's perfect when you really think about it.

Ilsa: She Wolf of the SS (1975)

The worst id probably Ellen Burstyn's character whose life revolves around a game show and how she slowly destroys herself as she trys to become a contestant. Wayan's and Leto's characters slowly kill themselves with drugs all the while dreaming of that one chance to become the big shots! Senseless (2008)

Greedy Guts (2000)

When a childless couple learn that they cannot have children, it causes great distress. To ease his wife's pain, the man finds a piece of root in the backyard and chops it and varnishes it into the shape of a child. However the woman takes the root as her baby and starts to pretend that it is real. When the root takes life they seem to have gained a child; but its appetite is much greater than a normal child. Another movie that has an ending in which you're telling yourself, "Nope, they're not going to go there" and then they do! Excellent cast including Jeremy Sisto, Anna Faris and James Duval! Fun Fact: The movie was original scripted and designed to be a cheap porn flick, but neither the producers nor Wes Craven were able to finalize the film in time.
